Expert care team
Bluff is staffed 24/7 by a team of psychiatric providers, nurse practitioners, registered nurses and therapists with many years of experience treating patients with substance use, pain and dual disorders. Pre-admission consultation
We welcome consultations with medical professionals, behavioral health professionals, insurance companies, healthcare organizations and other professionals who have a patient or client struggling with addiction. Members of our multidisciplinary team are happy to walk you through our treatment modalities in detail, and discuss any specifics pertaining to your patient’s individual case.
Coordination at intake
Our intake specialists will coordinate with you, your patient and their family to ensure a smooth, seamless and supportive experience upon arrival.
Ongoing collaboration
As the patient’s recovery progresses, we’ll follow up with you regularly – to seek out your input and perspective and to keep you informed of their progress along the way.
Continuum of care
We offer all levels of care for addiction to meet individual patient needs. Detox can be conducted at our residential center or inpatient/hospital for higher-risk cases, or can begin at home for patients who need to travel to us. To provide a seamless, patient-centered continuum of care, patients can step down from residential care to a partial hospitalization program (PHP), intensive outpatient program (IOP) or outpatient services. Our goal is to ensure patients receive the level of care that is right for them.
Aftercare planning
An integral part of our program focuses on relapse prevention and aftercare planning. When a patient is ready to leave our care, we’ll coordinate closely with you to review treatments, progress and recommendations for follow-up care. We also assist patients on obtaining secure housing such as a sober living home, and getting connected with support resources in their community, such as a sponsor and 12-Step programs. Our experts will continue to be available to you to discuss any concerns or questions.
After completing our referral form, a representative from Bluff will reach out to have the patient’s medical records transferred and confirm details of the patient’s situation. We will then contact the patient to follow-up and schedule the patient for an evaluation or admission to Bluff.
